Worplace Safety Briefing
IIF Data Solutions, Inc. is committed to providing a safe and productive work environment for all of its employees. In order to achieve this goal, it is important for each employee to understand how to promote workplace safety and prevent accidents and injuries.
We take this commitment very seriously, and as a result, we are requiring all employees to review the Workplace Safety Briefing. Once you have reviewed the briefing, you MUST complete and return the Workplace Safety Briefing Acknowledgement Form to hr@iifdata.com or fax to 703-745-3176.
This is a mandatory requirement for ALL IIF Data Solutions, Inc. personnel. All acknowledgement forms MUST be received no later than Friday, May 3, 2013.

2013 Open Enrollment
It’s time for our annual benefit plans Open Enrollment period. The continued rising costs of healthcare have forced us to examine our current benefits package in great detail. Tighter government restrictions and upcoming penalties under the Patient Protection and Affordable Care Act (Healthcare Reform or Obamacare) prevent IIF from continuing to offer the rich benefit plans that we have been accustomed to, at an affordable price. Instead we are forced to begin the transition to Consumer Driven Healthcare Plans (CDHP’s or high deductible plans). Unfortunately, these plans are becoming the standard in the healthcare industry. CDHP’s encourage covered members to take a closer look at where their healthcare dollars are being spent. Because insurance claims ultimately drive the cost of healthcare asking questions like “Is a generic prescription acceptable?”, “Would visiting an Urgent Care center rather than the Emergency Room be a better option?”, etc. can save thousands of dollars in claims each year. Please review the changes to our benefits package below. Coverage will be effective April 1, 2013. Deductions will begin on the May 1, 2013 paycheck.
We will be introducing a new online enrollment system for employee to utilize when electing their benefits. ALL employees MUST log into the system to elect or waive their coverage for the 2013/2014 plan year.
*Please go to www.mybensite.com/iifdata to access the system. The initial site username is: iifdata and the password is: benefits. This is a generic username/password that will allow you to access the basic plan information. Once you have logged onto site you may view the plan summaries and premium information.
*After you have reviewed the summaries and are ready to make your elections you will need to click on the “Enroll Now” tab. Your unique user ID will be the first 4 letters of your last name, then your 4 digit year of birth. For example Joan Smith born on 9/30/1970 = Smit1970. Your unique password will be your 9 digit social security number (with no dashes).
Medical: Our medical insurer will continue to be Aetna. We will be offering employees the option to enroll in one of 2 PPO plans; PPO 1000 or PPO High. We will no longer offer an HMO option. For the first time, we will be introducing a plan with a $1000 deductible. Please remember that under Healthcare reform preventative care is covered at 100%. Both plans will also offer 1 routine exam per covered member, per year and a $200 annual credit for contacts or glasses. Please refer to the summaries provided on the benefits site for detailed information on each plan. I have also attached a PDF document to this email that includes a brief summary for each plan.
Dental: Our dental insurer will now be Guardian. The annual maximum benefit has increased to $2000. Our new plan will also provide a $1,500 lifetime maximum benefit for both child and adult orthodontia care. Please refer to the summary provided on the benefits site for detailed information. Vision: Our vision insurer will continue to be Guardian/Davis Vision. Please refer to the plan summary of the benefits site for detailed information. I have also attached a PDF document to this email that includes a brief summary.
Basic Life & AD&D Insurance: Our Basic Life and AD&D insurance carrier will now be Guardian. Our plan offers a $50,000.00 flat life insurance benefit with an additional $50,000.00 benefit if the death is deemed an accident. This benefit is 100% paid for by IIF.
Voluntary Term Life Insurance: We will now offer voluntary term life insurance for employees, spouses and child dependents. Our carrier for this plan will be Guardian. Employees may elect coverage for themselves in $10,000.00 increments to a maximum of $500,000.00. They may elect coverage for their spouses in $10,000.00 increments to a maximum of $250,000.00, not to exceed 50% of their elected amount. Employees may also elect coverage for their dependent children in $1,000 increments, not to exceed $10,000.00 or 10% of the employees elected amount. This benefit is 100% paid for by the employee.
Long Term Disability: Our LTD carrier will now be Guardian. LTD coverage will pay 60% of the employee’s basic monthly salary, up to a maximum of $3,000.00 after he/she has been unable to work for 90 days due to injury or illness. Please refer to the plan summary of the benefits site for detailed information. I have also attached a PDF document to this email that includes a brief summary. This benefit is 100% paid for by IIF.
Voluntary Short Term Disability (VSTD) Insurance: Our VSTD insurance carrier will now be Guardian. VSTD coverage will pay 60% of the employee’s basic weekly earnings up to a maximum of $1500.00 per week. This is a voluntary benefit, paid for 100% by the employee. This benefit is 100% paid for by the employee.
Critical Illness: Our Critical Illness carrier will now be Guardian. Critical illness insurance allows employees to purchase coverage to help fight the costs associated with a life-threatening disease. Please refer to the plan summary of the benefits site for detailed information. I have also attached a PDF document to this email that includes a brief summary. This benefit is 100% paid for by the employee.
Flexible Spending Accounts (FSAs): Our FSAs will continue to be offered through CareFlex. The maximum yearly deposit amounts for the 2013/2014 plan year will be:
-Health Care Spending Account: $2,000.00; used for approved medical expenses.
-Dependent Care Spending Account: $5,000.00; used for approved dependent care expenses.
You do NOT have to be a participant in our insurance plans to enroll in a FSA. Enrollment in a FSA is only available during Open Enrollment.
-All employees electing to participate in this plan MUST complete a new form annually.
Once you have made your 2013 elections, please log onto the benefits site to enroll/waive the benefits offered.

Please remember that except in the case of a qualifying event, Open Enrollment is the only time you may make changes to your health care benefits. Qualifying events include: marriage; birth, adoption, or placement for adoption of a child; divorce or legal, court approved separation; death of employee’s spouse; change in employment status of employee or employee’s spouse; exhaustion of COBRA coverage under another health plan; loss of other coverage or a significant change in other coverage.

IIF Data Solutions 401(k) Plan Changes
As you have been previously informed our 401(k) Plan is changing! We are very excited to introduce our employees to CDM Retirement Consultants, Inc. CDM will serve as our plans Third Party Administrator (TPA). As our TPA they will be responsible for providing investment, administrative and record keeping service.
As a result of the TPA changing, the funds being offered in the plan will be changing as well. Please click on the complete memo below for the full list of funds and mapping information.

Education Meetings:
We have put together two enrollment and education meetings to learn about our new provider and discuss the new investment options and services that will be offered by our plan.
CDM’s Director of Business Development, Tom Rouse will be discussing administration, conversion and enrollment. IIF’s Financial Advisor, Bob Taylor will be discussing investment options and re-enrollment.

After registering, employees will receive a confirmation email containing information about joining the webinar. Employees will be able to log into the meeting using their home or office computers and either a headset/microphone or telephone to communicate. After logging in, employees will receive an audio pin, which will enable voice communication. It is important that employees enter this pin in order to activate audio for questions and comments at the end of the presentation.
